Health Information Management Department

The Health Information Management department, also known as the Medical Records Department, is dedicated to the integrity and security of your private health information in accordance with state and federal regulations. Health records are retained for a period of ten (10) years after your last date of service or final treatment.

How to Request Medical Records

Option 1: View Medical Records in MyChart

MyChart is a free online portal offering personalized and secure online access to medical records.

Option 2:Request Medical Record Copies Online

Request medical records via secure website hosted by our release of information partner, Ciox.

Option 3:Submitting Medical Record Request by Mail or Fax

If you do not wish to submit a medical record request online, please download and complete theConsent for Release of Medical Information form. Return the form completed by fax, mail or email including a copy of your photo ID.

Medical Record Copy Fees

A fee for medical record copies may apply. A patient whose records are copied and sent to another healthcare provider for the purposes of continuing medical care does not pay a fee for medical record copies. Request for medical record copies for any use other than medical care may be subject to a fee. The HIPAA Privacy Rule permits healthcare providers to impose a reasonable, cost-based fee to an individual requesting a copy of medical records. Patients or the patient’s legal guardian will be notified if any fees apply prior to the request being processed.
